My thought was to have it repaired, but I'm not an RTR enthusiast so I've decided part out. There are no scuffs on the plastic buttons, and block assembly includes linkage mounting screws as shown.
The heads have been sold, but at the time of this posting almost everything else is available. This may also fit other model's - the Akai stock number is BK536398 as far I can determine. Don't like the price. The face plates are all in clean, unmarked condition, but I have no way to test the electronics.
